Serves: Acclaim Health specializes in adult care and support groups.

Eligibility: Acclaim Health has several different services offered. Respectively, the eligibility for these support groups and services include the following situations. The individual is in need of Palliative care, is in need of symptom management, is suffering from Alzheimer’s (Dementia), is in need of home health care, or is in need of bereavement support.
